چکیده

PurposeBronchoscopy is invaluable in the diagnosis of airway complications, infection, and rejection lung transplant recipients, a fundamental skill pulmonology fellowship training. We aimed to improve bronchoscopy safety diagnostic yield our teaching institution by targeting two quality metrics: 1) higher patient radiation exposure fellow-assisted compared attending-only procedures 2) variability documentation exam findings.MethodsUsing an A3 lean approach, we assessed current conditions retrospective review bronchoscopies over four months. Then, established target ≤ 2 times that standardized per ISHLT grading. Next, conducted gap analysis identify root causes implemented countermeasures twelve months.ResultsIn conditions, mean was 2.6 ± 0.3 than attending-only. targeted fluoroscopy duration, lack knowledge, variable feedback with these countermeasures: tracking exposure, self-assessment competency, access education video, post-procedure attending feedback. As result, achieved 11/12 months (92%). In included ischemia, malacia stenosis < 20% bronchoscopies. knowledge creating templated report multiple choice options document grading placed representative graphics suite. dehiscence, malacia, were documented > 80% bronchoscopies.ConclusionWe successfully reduced difference fellow assisted implementing using approach. Bronchoscopy findings. Using

The diagnostic yield of CT-guided percutaneous lung biopsy in solid organ transplant recipients.

BACKGROUND Despite the widespread use of computed tomography(CT)-guided percutaneous lung biopsy (PLB) in immunocompetent patients, the diagnostic yield and safety in solid organ transplant (SOT)recipients is unknown. The purpose of this investigation was to determine the test performance of CT-PLB in SOT recipients. METHODS We performed a 10-yr single-center, retrospective analysis among hea...

Safety and efficacy of outpatient bronchoscopy in lung transplant recipients - a single centre analysis of 3,197 procedures

BACKGROUND Bronchoscopy represents an important diagnostic and therapeutic tool in the management of lung transplant (LTx) recipients. Outpatient bronchoscopy reduces health costs and may improve quality of life amongst these patients. This retrospective study assessed the safety and efficacy of outpatient bronchoscopy including trans-bronchial biopsy. METHODS All outpatient bronchoscopies pe...

Single-lung Transplant Recipients

• BACKGROUND Many benefits and adverse effects of positioning are related to changes in ventilation and perfusion. A number of unique factors related to the allograft make the effects of positioning difficult to determine in single-lung transplant recipients. • OBJECTIVES To determine the effect of 3 body positions (supine, lateral with allograft lung down, and lateral with native lung down) on...

Diagnostic yield and safety of electromagnetic navigation bronchoscopy for lung nodules: a systematic review and meta-analysis.

BACKGROUND Electromagnetic navigation bronchoscopy (ENB) is an emerging endoscopic technique for the diagnosis of peripheral lung lesions. A thorough analysis of ENB's yield and safety is required for comparison to other sampling modalities. OBJECTIVES To describe ENB's yield and safety profile. METHODS The MEDLINE and EMBASE databases were systematically searched for studies reporting ENB'...

PREGNANCY IN RENAL TRANSPLANT RECIPIENTS

ABSTRACT Background: Correction of the uremic state by a functioning allograft often restores fertility in women of reproductive age. The rate of fertility significantly differs between industrial countries, developing and middle east countries. On the other hand the results of pregnancy in Kidney Transplantation (KTP) patients are significantly better than hemodialysis patients, and pregnancy...
